Printed are available UPON REQUEST by the employee. To request a PRINTED 1095-C form, contact Kathryn Sarvak via email: ksarvak@champlainfarms.com or by regular mail: 32 San Remo Drive, South Burlington, VT 05403 or by phone (802) 864-5155 ext 250. Be sure to state you are requesting a printed 1095, and include your full name, and last four of your SSN clearly.
Employees wishing to go paperless for all tax forms may do so in ADP at Myself>Pay>Pay & Tax Forms. Scroll down to the Go Paperless info box. This will include your W-2 and 1095 C.
