ACA Notice of 1095 Furnishing

Printed ACA 1095 C forms will be made available ONLY UPON REQUEST by the employee.

Printed forms will no longer be automatically provided to eligible employees. Consistent with federal law, any employee wishing to receive a PRINTED copy must request this directly with the main office. See contact info below.

Alternatively, you may elect to go PAPERLESS for your tax forms. To do so, go to Myself>Pay>Pay & Tax Forms and scroll down to the Go Paperless info box. This will include your W-2 and 1095 C.

Be sure to state you are requesting a printed 1095, and include your full name, and last four of your SSN clearly.
